FIELD NAME DATA ENTRY REQUIREMENT
Name: Enter your name and surname
Email Address: Enter your primary email address
Domain Name: Enter the URL of your main domain name.
Month/Year of launch: Enter the month and year you launched the site (mm/yyyy).
Product type: adsense / ebook / audio / video / dropshipped physical product /etc (enter more than 1 if required).
Other content: What other content do you offer as give-aways or sale incentives? (eg. 5 ebooks, 2 audios)
Monthly income: Enter the monthly income in US$ for the site.
Monthly Profit: Enter the monthly profit for the site in US$.
Monthly Ad cost: How much money (US$/month) do you spend on advertising?
Daily Traffic: How many unique visitors are visiting your website per day?
Main keywords: Enter the main 5 or 6 keywords (separated with commas) that you are targetting for your website (max 100 characters available for entry).
Google pagerank: Enter the average Google pagerank for your site.  If you don't have the Google toolbar installed then you can get the Google pagerank by entering the URL of your website at this website:

Number of backlinks: Enter the number of websites that are linking to your site. You can get an idea by entering the command:
link:www.yourdomainame.com into Google. The number of pages that appear is the number of backlinks that Google has found to your site.
Weekly maintenance time: Enter the number of hours per week that you spend managing and maintaining your website.
Number of pages: Approx. number of pages of original article content on site.
Squeeze page: Have you set up a squeeze page (enter yes or no).
Number of opt-ins: How many names/email addresses do you have in your opt-in list
Split testing: Have you set=up any split testing on the site (enter yes or no).
Number of affiliates: How many affiliates do you have selling your product.
Non compete agreement: Are you prepared to enter into a non-competition and non-disclosure agreement with us.  This would prevent you from developing a website for the same product niche in the future.
